Job Description

Community Care Worker I Job Description:

  • Performs entry level case management work for Community Care Services Eligibility (CCSE).
  • Determines eligibility for CCSE program services.
  • Authorizes and monitors CCSE program services.
  • Interviews elderly and/or individuals with disabilities.
  • Documents information manually and into multiple automated systems.
  • Verifies case information utilizing multiple automated systems.
  • Explains program benefits and requirements.
  • Refers individuals to other programs and/or agencies.
  • Completes special assignments and/or reports.
  • Maintains confidentiality of all incoming and outgoing information.
  • Understands and adheres to all HHSC and CCSE policies and procedures.
  • Provides outstanding customer service in person, over the telephone, and via e-mail.
  • Maintains a positive and productive attitude while working in a fast-paced environment.

Essential Job Functions

  • Attends work on a regular and predictable schedule in accordance with agency leave policy.
  • Performs other duties as assigned.
  • Interviews applicants, consumers, and resource persons and conducts home/site visits to assess the need for services.
  • Works effectively with consumers, their families, provider agencies, and other state agency staff to compile information and ensure services are authorized appropriately and accurately.
  • Conducts reviews to determine and re-determine eligibility.
  • Obtains, verifies, and calculates income and resources to determine financial eligibility.
  • Documents information manually and into multiple automated systems.
  • Develops, coordinates, and explains client services and plans, and authorizes services to help meet an individual’s needs.
  • Serves as a consumer advocate and agency representative in locating available resources and making referrals to community resources, when appropriate.
  • Prepares basic-level special assignments, narratives, and statistical reports.
  • Monitors services provided by contract agencies to measure the effect of services delivered and to detect any problems.

Knowledge Skills Abilities

  • Ability to effectively communicate orally and in writing.
  • Ability to set priorities, establish timeframes, and meet deadlines.
  • Ability to establish and maintain effective relationships with individuals receiving services, co-workers, contract agency staff, and staff from other federal or state agencies.
  • Knowledge of interviewing techniques to obtain personal information, to make inquiries, and to resolve conflicting statements.
  • Knowledge of resources that serve individuals who are older or have a disability.

Registration Or Licensure Requirements

A current valid driver license.

Initial Selection Criteria

  • Prefer education and/or experience indicating the individual possesses the knowledge, skills, and abilities necessary for acceptable job performance.
  • Must be willing to travel at least 85% of the time.
  • Prefer typing at least 30 wpm.
  • Experience with Microsoft products such as Word, Outlook, and Excel.
  • Employment application illustrates knowledge of business English to compose correspondence, document, and record information.
  • Ability to operate a computer and general office equipment.
  • Experience with the aged and/or disabled preferred.
  • Experience interviewing, documenting, and reporting information preferred.
  • Experience communicating effectively with a wide variety of people in different types of situations/environments.

Additional Information

  • This position will be located at: 1067 Bandera Rd., San Antonio, TX 78228.
  • Applicants may not have a history of substantiated fraudulent activity against HHSC or any programs it administers. Applicants who have a non-fraud overpayment with an outstanding balance must agree to repay the overpayment balance as a condition of employment.
  • This position is included in a career track series and will be filled at the Worker I level unless the selected applicant’s qualifications meet career track promotion eligibility requirements. Applicants with previous employment with CCSE/CCAD will be reviewed by program management to determine appropriate entry level.
  • Must be able to attend training which may require overnight stay.
  • At the point of selection for an interview, the most recent performance evaluation may be requested.
  • Must be able to conduct home visits in an assigned territory which could be within a 100-mile radius from headquarters.
